This form is intended for adult (18+) members who are participating in an approved third-party fitness or recreation program that is hosted at The Club.
Once the form has been submitted, you will be directed to a page to pay the annual membership dues. If you prefer to pay offline by cash or check, please see the contact information below. Once your enrollment is approved, you will receive an email confirmation. The membership period is July 1 through June 30, and must be renewed annually to continue participating.
